Overview

<p>The Foundation's grant-making program is centered on the concepts of health and well-being. The Foundation's purpose is to promote and support effective and creative programs, practices and policies related to healing from illness, accident, physical, social or emotional trauma and to extend the availability of programs that promote healing to underserved populations. The Foundation is particularly interested in funding programs that address the health of individuals who, because of barriers to accessing care, experience poor and sub-optimal health, including: those with no or severely limited income, cultural differences, lack of English language skills, lack of health insurance or inadequate health insurance, limited access to health care services, mental illness, substance abuse, homelessness, incarceration, and exposure to trauma. <br>More specifically, the Foundation favors proposals that seek to promote healing, healthy lives and healthy communities through: <br>- Innovative demonstration projects that address a major gap in the field <br>- Outreach to communities and populations whose health care needs are not being met <br>- Targeted advocacy efforts for vulnerable populations <br>- Collaboration among providers and other organizations <br>The Foundation is interested in projects that seek to improve the physical and mental health of individuals involved in the criminal justice system. Programs focusing on alternatives to incarceration, detention, and reentry will be considered. While the Foundation does not support prevention projects, it is interested in programs that seek to reduce recidivism. <br>Chronic Violence and Community Health: <br>The Foundation views violence and its impact on communities as a public health crisis. The Foundation is interested in projects that seek to end the cycle of violence at a community level using a public health approach. Programs focusing on fostering collaboration between agencies, advancing policy reform and systems change, leadership development, and coordinating public and private funding streams will be prioritized. The Foundation does not provide operating support or funding for program services.</p>

Solicitation Limitations: <p><span>The Foundation’s grants are directed principally to activities conducted in New York State. Exceptions to this geographical limitation may be made for proposals that hold promise of national impact or extensive replication.</span></p> Other Information:<p><span>The Foundation will support projects from outside New York State. However, to be considered for funding the project must hold promise of national impact or extensive replication.</span></p>
